IMPORTANT!! PLEASE READ THIS BEFORE PRINTING APPLICATION!!
Registration
There is a Code of Conduct form for both swimmers AND parents to sign. This form should be printed and included along with your registration form and volunteer commitment contract. Fill out all forms completely (please remember to list all/any allergies separately for each swimmer). As usual, the volunteer commitment contract needs to be accompanied by a separate check (one commitment check per family). This check will be returned once your commitments are fulfilled. If we receive either an application without a check, volunteer commitment contract without a check, or Code of Conduct form either missing or unsigned, your swimmer(s) will not be registered. We must receive all forms postmarked no later than 3/31/10. This is extremely important so we know how many new swimmer registrations there are. If we do not receive your completed registration on time, your child will not be listed on our roster.
Once your registration is received and processed, your child's name will appear under the 2010 roster on our website: www.quakertowndolphins.com. This will confirm that your registration was received, accepted, and completed. Your child's age as of 6/1/2010 will determine their swimming age group for the 2010 season.
Once your child is registered you will receive a confirmation e-mail listing both your login id and your password, which is the same as last year. Remember, you must be a registered member to view your child's stats and to sign up for jobs.
Once your swimmer's name is listed under the 2010 roster, you may immediately sign up for volunteer jobs. You will need to login first, and then you will have the ability to sign up for your positions. In addition, you will also be able to delete your name if a problem arises or if you have a scheduling conflict. However, you will not be able to make any changes two days prior to a meet. If an emergency arises, please contact June Hunt at huntclub2@verizon.net and she will make the necessary changes.
No more than two prepared food items (fruit salad, pasta salad, etc.) can be used to fulfill your volunteer commitment. Please note that in order to accommodate a wide variety of schedules, we have delineated the volunteer positions based on what you can do at home in contrast with what needs to be done at a meet.
Thank you.
